In my case, i opened a bank account using my passport and EP and it doesn't require any residential proof. As for broadband, i don't remember they asking for proof as well. What the telco told me is that, they will post me a letter and i have to send it back to them just to prove that i've received the letter and i'm staying there (which i didn't post back anyway.. so far still ok). For other services, you can always use your bank statements as proof.

EP doesn't state your residential address, not sure about the old ones though.

Bro, why do you need to make payment for card collection? I don't recall any charges apart from the medical check up which can be absorbed by the employer if you request a medical chit from them.

As for the stamping, i don't quite get your situation...
*
u need to make payment for the card.. $30 per year which is paid by the employer via the EPOL

the stamping issue is that I left 1 day to access Singapore and half a page left for my passport.. lol

Yes, but bear in mind that once the extract is generated, the JPJ officer may terminate your license in his system too.

There are actually 2 way to get both Singapore and Malaysia licence valid at the same time.

1. Pay RM300 to the runner, ask him to settle for you.
2. After convert Singapore licence, go LTA/TP get the similar extract, go back JPJ say want to cancel Singapore licence, the JPJ officer will cut your Singapore licence into half, and issue you a new Malaysia driving licence. Then go back to LTA/TP to get the new Singapore licence, pay SGD50 I think.

Yeap, just say you need driving record extract. Normally they don't ask why. I just renew my driving license, no problem no black list.

So, it's called "slip catatan lesen trafik"? I had asked for a "surat" to give to Sg traffic police to get Sg licence. And they came back (both times) saying that they needed to blacklist my licence.

U went to JPJ in JB? Better u go to JPJ in some kampung areas. U might get yourself lucky sometimes for not being blacklisted.
