AFAIK, this method only applies to the 2G era where ie week 45 onwards if 1.1.2 firmware with 4.6 bootloader.
erk, i dont have any link, hehe sorry, it is because from my experience on unlocking iphone for my brother and his friends, my brother is KLIA cabin crew so he usually buy iphone from UK, and my experience only base around 4~6 iphone 3G that i have unlock with yellowsn0w and some of my brother friend who also ask me to upgrade firmware and unlock it.
i usually see their production number and all iphone around year 2008 will be maximum on 2.2 firmware and the baseband will below 2.28.00 and some of the phone that early january( between week 1 to week 5 of 2009) also have 2.2 firmware. And it was on valentine day ( week 06) 2009 my brother buy another 8gb 3GB iphone and its preloaded with firmware 2.2.1 baseband 2.30.00.. . he still buy 2 more( after a month, around may) and still have same firmware, i check the serial number and i saw after week 6 of 2009 ( around 14hb of february) the firmware was preloaded with 2.2.1 and as a result all 3 of my brother friend have to use turbo sim in order for them to make a call etc..
im not sure about others country locked phone but then phone i was mention is from UK and the phone my brother bought is PAYG iphone.
regarding apple refresh all firmware into 2.2.1 without see its week production or not im not sure because usually when 1 product is build up, they will be preloaded with the latest firmware and firmware 2.2.1 was publish/release end of jan so all iphone that come out from factory will be preloaded with firmware 2.2.1...( i believe only the iphone that have preloaded with firmware 2.2.1 and its week production around 2008 then i think it was refurbish unit< -- please someone if im wrong in here, please correct me)

and..urmm.. i afraid i can't answer that but from my understanding in reading serial number, digit 3 is for the year production and digit 4 and 5 is week production.
example.
Serial number: XB739XXTWXN
the 3rd digit represents the year ie 7 for 2007
the 4th & 5th represent the week ie 39 for week 39
all the rest indicate the factory, or whether it's a refurb, etc.
as for my iphone is:
85835xxxxxx where i buy my iphone last year early october( before hari raya) and i know that my iphone is new base on itt week number.
