Highly suspect is one of the reason your shrimp died.
Check nitrite if tsnk still new.

Leech are hitchhiking on plants and sometime on shrimp you bought.
They won't suddenly be born out of the soil. Feeding live food is one of the possible way to bring them into tank too.

Need some advice from the Sifus out there....
I am using Seachem Prime, which will provide a natural slime coat to the water...
However, I am having these white particles on the surface of the water...

Anyone facing the same problem before?
Is it a good thing? Or should I remove those white particles?

i using Seachem prime for almost 3yrs, never encounter those white particles, those slime coat is mean on the fish body.
Did you check if those white dust coming from your ceiling board ? or other place/stuff.

Your moss look yellowish, not healthy, can due to high temp, not enough light or fert.
