Since I need this card badly, so supp-ed this card for them. If their groceries is heavy, then maybe will think of Ikhwan which I wanted earlier but they said Fridays and Saturdays not feasible.

what should I fill in the Customer’s NRIC / Passport No. / No. K/P / Paspot Pelanggan : bottom of each page?

I filled in with the supp card number, and my cards were approved.

Quite annoying right? Haha.

I filled in mine.
My office's Adobe Acrobat Reader allows typing and editing, so no issue for me to type and copy.

One using supp id, one using principle id.
I think as long as all page is the same id should be ok already.

Btw, the sms transactions alert and TAC for suppcard will be send to supp phone or principle phone?

I leave it empty. CIMB officer called to verify my application and I asked about it, he said can leave empty and OTP will be sent to principal.

do you mean leave the supp contact no empty?
they no need to call the supp person to verify?

Yes, left it empty.
They will not call. So far I heard only AmBank called supplementary card holders but that also they didn't call my supplementary to verify.
