So it excludes your pre-existing conditions and on top of that charge you more...

That's definitely new to me.
I doubt that is the modus operandi anywhere in the industry.

Did anyone's premium increased suddenly? Mine increased from RM300 to RM323 per month starting from May 2021. Agent told me because of medical card repricing due to increment of medical fees.

form last year , this year , and i forsee as long as covid still there
insurance company will keep on increase premium
because too many people claim

It is better for you to review your policy to understand all the coverage under covid , what to add , what to reduce or adjust your risk accordinly.
if you do nothing , you might keep on getting premium repricing letter.

It's due to Medical Inflation, I hope your agent didn't mis-inform you that your insurance premium is "guaranteed fixed" the same throughout the tenure of the insurance policy when you sign up for the policy.

Sustainability you can edit / adjust to 60,70,80,90,100 , the higher the more expensive. you have to think yourself you would like to cover until which age first.
PRUDisability Provider Benefit - you can either remove or maintain this , most of the people maintain it as this is quite a good rider & cannot be found in new policy anymore. Once remove it cannot be added anymore in future.

Based on my opinion , suggestion , you can adjust
Change Life from 160k --> 100k
Increase Crisis Cover 80k --> 100k
PRUDisability Provider Benefit --> 20k (Maintain)
Accident -->100k
Clinic Accident Claim --> 2k
PruMajor Med --> upgrade to Pru Million Med
Sustainability --> 80 years old

and check whether you can afford the budget or not, from there then you adjust.
If the premium is quite cheap and you got extra budget
Option 1 : Upgrade Critical Illness to Total Multi Crisis Care
Option 2 : Increase sustainability to higher age

If the premium is over budget , you can either :
Option 1 : reduce sustainability
Option 2 : maintain prumajor med card , dont upgrade to pru million med

*Above are just merely general suggestions , if you have very specific own requirement then its gonna be a different story.

Everyone's need is different depending on your existing circumstances, talk to your agent and find out what you're lacking of and get a proper financial health check rather than getting blind suggestions. You don't see doctors giving out medication without first finding out what's wrong with you.

It depends on the type of plan you are going to subscribe to. If it is an investment linked product, then no, there is no discount

However if it is a traditional plan (mostly for savings only) then yes. I suggest getting a FULL Quotation from the agent you're subscribing to.

In the quote it will mention if there are discounts depending on the frequency of payment :- between monthly, quarterly, half yearly and yearly.

May or may not. It's all up to the insurance company when it comes to the revision later on.
