Hi guys, if I am currently under hire purchase agreement with public bank with very good track record of servicing the loan, do I need to submit additional income proof to apply for a credit card ?

Only the bank would be able to answer this. If you can provide the income docs, then just submit.

Is the annual x3 ppl access by calendar or statement year? For the travel insurance, if I am purchasing the flight ticket for 5 people under this card, is everybody covered or only the cardholder will be covered?

You need to read the policy. Some covers immediate family members so check on the definition.
