I maybe wrong, but this is what I did previously:
a) Consider as self-employed, but no matter what, just fill up the total gross income into Penggajian column. Forgot I use BE/B, but no issue from LHDN.
b) Yes & No. Also depends on your contract / hiring with your overseas company. My arrangement is to submit my own EPF (both employee and employer) and claim the amount in next month from company using the EPF receipt.
e.g. If I'm being paid TT RM10k basic salary a month. I deduct 11% (employee) myself. Then I fork own own money first to pay 13% (employer). My first month income is 10k - 1.1 - 1.3 = 7.6k balance.
every subsequent month is 10k - 1.1 - 1.3 (current month) + 1.3 (claim for previous month) = 8.9k nett which is correct.
Total income is 10k + 1.3k = 11.3k

Of course, if you contract/package is nego to be EPF Employer inclusive, then is up to you.
If I don't claim Employer EPF, I will lose out 1.3k every month.
First month is difficult since need to fork out additional money, but after several months, the money keeps rolling.
If you pay EPF, you can get deductions from tax submission.
