No particular template but maybe write an email first to the following persons in the property management and customer service. Poor Customer Service.

Mohd Yusof Saidu Mohammed <yusof@nazattdi.com>,
Muhamad Zaki Muhamad Amir <muhamad.zaki@nazattdi.com>,
Rizalludin Hassan <rizal@nazattdi.com>
TTDI Alam Impian residents must stand united and give them some pressure. They are in fact earning tens or hundreds of millions of dollars from the project.
First Serve - as for your defect lists, you need to follow up closely and if required, ask to meet the contractor together with the customer service person handling the defect issue. Point out each and every defect. Best to mark the problem using a market pen on masking tape and stick it near the problem area, and take photos for your reference and email to the person in charge. Also, when you submit your defect list, make sure that they chop and sign and give you back a copy with received date.
Platingirl.. Yes i have written to two person from the property management department, as per the handbook given. Thank you for the list of names, i shall forward my case to them as well. By now, the contractor would have already painted over all the water marks. I will need to have a joint inspection after a couple of heavy rainy days... definitely the marks will be back. mohamad.asri@nazattdi.com

It will be a shame for them to let such small issue stain their reputation...
