it's best to go through a tour agent, just like me and zoakies. less hassle for just a small fee. i did mine with:
1) CONFIRMED (i.e. booked and fully paid) flight tickets (although not necessary now, but should increase ur chances of successful application)
2) Bank statement (don't worry about amount once you have your tickets. i had way less than RM10k, although combined with my wife's statement, it exceeds RM10k but i dun think it matters coz it's all individual applications, regardless of relationship)
3) Official letter from company. (If ur company doesn't know how to write one, there's a sample in English on Singapore's Japanese Embassy website. i followed that sample)
4) 1 passport-sized photo (of coz)
5) Official application form (duh)
As for the itinerary and accommodation, i let the agent handle it. Till now, i still have not booked my accommodation. For those who are married and going with the wife, my agent recommended giving a copy of ur marriage certificate. She says with that document, applications are rarely rejected. So i gave her a copy too.
