It seems that your workouts are too gunz-oriented. You can do wonders with Single Leg Split Squat (AWESOME exercise), Pushups (on the floor/dinding/kitchen cabinet/chair etc - one arm if you think normal pushups are too easy),Dips in between tables/kichen cabinets, Deadlift with DBs, upright row with your cooking gas tank and lots of other kick ass exercises with just db, your bodyweight and other household equipments

You can keep your cardio if you like but you gotta eat way more than what you're eating now. Bump your protein intake. By a lot. Have lots of eggs/milk with your sandwich for breakfast if you dont like to cook. Your diet has a lot of processed food - learn about nutrition a bit, read labels and you'll be surprised what crap they actually put in your food.
But hey at least you do your cardio dues

alot of processed food? yea..but its just the tuna actually...apart from chili sauce and all.
Dinner is usually home cook food.....coz my grandma stays with me...veg....chicken...pork and soup at times.
I usually have a small serving of rice and more veg and meat.
