The account number of My TMNet Streamyx (bundled with TM fixed line) starts with D. Just to confirm, I should select Telekom Malaysia, in UOB PayOnline, right?

Check your bill. Whether printed on the bill is TM logo or Streamyx logo. Pay according to the logo on your bill.

do you know UOB bank forex rate is how many (website or something)? Because wanna tie it on my PayPal, so when buy stuff in USD I can roughly know how much.

I don't know and I never check this kind of thing before... If you have CIMB CR and amount lesser than RM 600, you can consider CIMB CR as it has lower exchange rate.

Eligible transaction must meet RM 50 min in order to get 5% cash back on the advertised category.
If your transaction < RM 50 on the advertised category, it won't be eligible to get the 5% cash back.

The maximum cash back is capped at RM 50 which in turn translated to RM 1000 of spending on on the advertised category monthly.

You pay at where? UOB VOX CC cash back credit system is not consistent to other cash back cards in the market or by UOB.
