adb reboot bootloader
At this point, phone in green-android-mode.
fastboot flash bootloader bootloader-mako-makoz30f.img
If you see something like device not found, etc at this point, that mean your adb drivers is not the Universal driver. Follow these steps to make it Universal (https://forum.lowyat.net/index.php?showtopic=3345520&st=600&p=71498268&#entry71498268) or download it from XDA.

The last fastboot is without "-w". That will keep all your data and settings. You will lost Root and custom recovery. Easy to get it back.
