The norm of address woman based on the marital status is no longer applicable nowdays. I remember reading the article about the woman in western countries, they are no longer want to use their husbands' family names because it has been practiced since medieval ages where women have no will and power to decide anything on themselves. Same goes to the salutation of ms or mrs.

If in business or formal setting, call puan. Whether the person is single or married is irrelevant, puan is to show respect. Use cik if only the person told you to call her cik instead.

In casual/informal setting, if you know the lady is single then use cik. If unsure, use puan first. Better to start on more formal then lower down.

For surgeons, they are informally addressed as Miss or Mr instead of Dr. So yeah i guess Miss is more atas.
