Well, if design is such a big concern then you might want to consider
EON Bank Combi Mastercard or
UOB Bank debit Mastercard. Both cards can be used online but then if you want to add these cards into your PayPal account to withdraw money from it then Maybank and Public Bank would be your only choice as you can only use your VISA debit or VISA Electron to withdraw money from PayPal. Mastercard is not supported.

I personally think the free quota system given by Maybank is very fair. Why you might ask ? Because for all other banks we need to pay RM8 per annum for their ATM cards. Maybank however is free and if you might need to use it a lot but you do not exactly have much money inside your Maybank account, then you can always choose to pay RM8/pa and withdraw cash from any Maybank ATM as many times as you like regardless how much your account balance is.
Besides Maybank, Citibank is the only bank that do not charge an annual fee for their ATM card but then finding Citibank ATM in Malaysia could be quite a task.
