Sorry for asking again. I have three questions.

1. May I know exactly what is this document?

Borang Permohonan Online MyMaster/MyPhD yang telah diitandatangani

Below is the attachment of the checklist of documents to be submitted to MyBrain Unit KPT.


2. And do we have to print out the documents in colour or black and white will do?


3. Now the MyBrain website has issued a new version of the Borang Pengesahan Maklumat. So now, should I print out the new version, fill it in, get the necessary signatures again, and submit the new version or should I submit the old version which I had uploaded online earlier? Anyone experience this before?

(1) You should have retained the digital copy of the Borang Permohonan Online MyPhD (a.k.a Permohonan Program Pembiayaan MyPhD) when you first submitted the online application. Just print it out and sign over it.

(2) Black and white should suffice. However, color can differentiate between the original and photocopy, if you intend to make one for reference.

(3) Submitting the uploaded Old Version signifies originality that you don't alter anything. If there is no significant difference between the old and the new one, then I don't see any reason to trouble yourself to find your employer. Perhaps, there are new instructions you need to comply in attaching certain types of supporting documents.

P.S. The New version is probably for those who wants to make New Online Application. Since you have made the online application earlier before the changes, you should be exempted from refilling the New form.

(3) There are additional requirements in the new version of Borang Pengesahan Maklumat. Examples of the additional requirements are:- You have to get an HR officer in the IPT to acknowledge that you are not a full time staff in the IPT; you have to get your referee to put his/her signature down; and one has to submit the EPF statement together. There were no such requirements in the old version. Also, they shortened the requirement that the referee should know you at least for five years, down to three years.

I wouldn't have problem getting the signatures of the relevant IPT officers again as they are physically there in the University but to get the signature of my referee would be quite a trouble. And if I'm required to fill in the new version, I would have to change my referee.

I hope I am not required to fill up the new version.

I called up MyBrain a couple of times today but no one picked up.

Since this is a festive week, most likely you won't get response from the MyBRAIN Department. However, to simplify the matter, I'd suggest you to request the HR Officer from your IPT to endorse the Old Version, and attach the EPF Statement. Therefore, you will solve 2 problems in 1 move, complying with the New Version without troubling your Referee. Moreover, your original Referee's Report is valuable as white truffle.

May i know,where did you get the "senarai semak doc"?...i've got the offer,downloaded all the doc, but didn't found that senarai semak..

It's at the MUATTURUN site which you can see it on the top mainpage of the website itself.

Anyway, did you fill in the old version or the new version of the Borang Pengesahan Maklumat when you uploaded online earlier?

i fill in the earlier version..since i was in "graduan tak bekerja" category..i didn't face too much problem to fill in the new version..but fill in the new borang pengesahan is like doing the same thing twice..and they only give us 14 days to settle everything~~huhu..

So that means you filled in the new version of Borang Pengesahan Maklumat and getting all the signatures again?

in the Mybrain website, they didn't say anything that you can sent them the old version..that's why i fill in the new form..

It seems like they want us to submit the new and the old version together because the Borang Pengesahan Maklumat is mentioned twice in that login webpage (after we have logged in our ID).

They also mentioned No 6. Kajian Cadangan Penyelidikan (Lampiran B) - jika berkaitan. I have no idea what is this.

If they already mentioned No 3. Rancangan Penyelidikan (proposal) yang telah disahkan oleh penyelia, then what is No 6.?

i have few questions regarding MyMaster program. i received the offer letter on 12th nov 2012 and need to submit it within 14days from date of letter

1. in the offer letter it left out the name of my course. all of the details are there except KURSUS: (blank). is this normal or something is wrong here? all my name, my address, my mykad no, my UNI are there.

2. where should i do my medical checkup, is it compulsory to do only in Gov clinic/hospital or can do in any private clinic?

i tried to channel the question right to kementerian (through phone number provided) but several tries fail, cant get through at all.

so for those who already got and currently on that offer please help enlighten me. thanks

Someone asked the same question in the MyBrain Facebook site.

The reply was if you were not asked to fill up the Borang Pengesahan Maklumat when you applied a few months ago, then you do not need to fill up now.

The new requirement is only for the new applicants.

And I found the answer to my question too. Someone asked the same question in Facebook.
Those who filled in using the old version of the Borang Pengesahan Maklumat have to send back the old version. No need to refill the new version.

is it the borang pengesahan maklumat or borang pengesahan status is for those who apply myphd only?
why i didn't asked for filling this form when applying mymaster?

I believe new MyMaster applicants now have to fill in the Borang Pengesahan Status/Maklumat.

If you did not fill in previously when you applied online, then you're exempted from it. This is the reply I read from facebook.

But then there was another reply saying that the requirement for MyMaster applicants to fill in that Borang Pengesahan Status has been enforced since August. Maybe a different officer said this.

Then I'm not sure already.

By right, if you were not asked to fill in when you applied, then no need to fill in.

Anyone know that for myphd, is there any bond need to be tie? When it stated must work in company with benefit of Malaysia, is it refer to government sector or any private Malaysia company also counted ?
