Looks like I'm answering my own thread. Finally opted to go with Dr Sushil Brito of SJMC. Very down-to-earth doctor from first impression, and although seemingly full of energy and busy with patients keep calling him on the phone including one Datin who asked to see him, he took the time to explain all questions I asked, rather patiently unlike some doctors who will rush it through and not seemingly interested to explain in detail especially Dr Koay Cheng Eng. Recommended, based on first impression and demeanour shown.

I not sure is there any one still look at this post. I just try my luck. Would like to get some opinion. I visited Gleneagles, Dr Tham, he told me my throat have a mass, ask me proceed operation to remove it. He insist for it.
Then I visited Dr Zul in UM, he told me just swelling. Not a mass. I also confuse. What should I do now ?
Each visit cost me around 1K. My company outpatient visit only 3K limit.

Somebody suggest me Prince Court, Dr Yeo.
I also see this post, seems UKM also a good choice.
Would like to have suggestion.

anyone had difficulties in breathing through mouth? i need to take a deep breath till it feels like reaching my throat only can pass through something. sometime feels quite suffocating and i not sure whether due to sinus or what but i am having sinus. my left side of nose is blocked and went to see general doctor mentioned is swollen. What should i do?

Hi, I need to do a biopsy to see if I have NPC cancer, can anyone suggest which government hospital is the best place to go in terms of cost/efficiency?

About 8 months ago, i had my upper molar extracted due to infection (with visible abscess)

Then about 4 months ago, I went for a CBCT scan to prepare for my sinus lift surgery. Dentist noticed that my right maxillary sinus was filled. Dentist advised to postpone the surgery, then prescribed anti-biotics.

After a month, I went back for a x-ray scan. the opacity of the right maxillary sinus was still there. Then I went to a ENT at Putrajaya Hospital who did a nasal scope. She mentioned that there's nothing out of the ordinary but I will require a CT scan to get the full picture. \

A week later I went back to the dentist and from the CBCT it still shows that the right maxillary sinus is blocked. Then I went to another ENT clinic and the doctor there said that from nasal scope he noticed that the right side of my nose is inflamed. He prescribed Mometasone spray, Desloratadine and ask me to perform sinus rinse once a day.

Ever since the upper molar was extracted, I didn't have any symptoms of infection. In the morning I do get a bit of clear mucus, but other than that I can breath normally.

Anyone has experienced this before? Why is my right maxillary sinus obliterated (filled) and any latest recommendations (I have read through the entire thread) of any good ENT for diagnosis?

I don't know how I missed your post. Regarding Dr Koay Cheng Eng, he gave me a bad impression when I visited him about 8 or 9 years ago. He seems to have bad character and very pushy, showing all the photos of his previous patients whom he has operated. He was pushing for septoplasty surgery, but when I asked him on the cons especially side effects, he immediately got agitated and didn't seem to be interested to entertain me anymore, gesturing to his nurse to take in the next patient into his room. You can refer to an older post made by another member here who had a poor experience with Dr Koay Cheng Eng.

I had since completed the septoplasty surgery for my deviated septum as you have experienced as well several years later with Dr Sushil Brito, details above, and it was a pleasant experience right from consultation to surgery and follow-ups. There was certainly an improvement after the surgery but it takes time for the breathing or air flow to improve as the effects are not immediate. I could expand on this but it's not related to this conversation so I'll leave it as that.

I'm sorry for your case. As with any operation especially something which is related to the nose where it can affect breathing which in turn is closely related to sleep and life quality and in your case tinnitus, it's considered major or big decision to go with the operation or not. As I have mentioned elsewhere, if the symptoms are minor and not threatening and you can still live with the condition, then bear with it and drop the surgery. You only opt for surgery if the condition is affecting your daily life and bringing difficulty to you such as problems in sleeping well and regular migraine or headaches etc.

With regard to tinnitus, what the doctor said is true. Tinnitus or ringing in the ears is something that is permanent, and I believe most people will experience it. It's either a mild or more severe condition as tinnitus will usually be more prevalent with older people, or people who often listen to loud music or are constantly exposed to loud sound in their work environment. I have tinnitus too and I could live with it. It's actually quite normal and I would advise that you try to adapt and live with it. Think of the condition being normal instead of it being abnormal or a disease. If you could not accept the condition as you have suggested, I presume it has created anxiety to the point you had given up your job. Just accept it and treat the condition as normal and you will be fine.

By the way, tinnitus is closely related to the nose. It's all inter-related as with ENT, ear nose and throat. If the passageway of one of the nostrils is narrower than the other one, a condition called deviated septum, it will affect the hearing with one of your ears too as the hearing will not be balance between the two ears. You can do a simple experiment by blocking the ear passage of one ear with your fingers when listening to music, then do the same with the other ear. The sound quality can be rather different in the frequency spectrum.

Total time spent maybe <5mins, damage of RM460.35.
Despite some positive comments about him in the forum, I think KCE is just impatient, not willing to explain anything at all to you, and just wanna see maximum patients in the shortest time, unless you are a potential surgery candidate.

Could any one recommend any better ENT specialist?

Hello, I am not sure where are the positive comments you've read about Dr Koay Cheng Eng as most if not all of the comments on this doctor are negative, including from me.

A good ENT doctor that I can recommend is Dr Sushil Brito of Subang Jaya Medical Centre, but he is not cheap, SJMC is generally more expensive than Sunway doctors. He will surely have the time to talk and explain everything to you. What you said about Koay is true, he's not only impatient but rude as he wants to see most patients hence he doesn't spend much time with the consultation. Reading all the responses here about him, I guess it is a norm for him to chase patients out of his room already. biggrin.gif

In my latest visit to Dr Sushil Brito, a consultation and bottle of throat spray costs me RM659.

By the way, just a sharing for people who have done septoplasty surgery to correct a deviated septum.

In my case, the surgery was done at the end of 2023 and it was successful. It was not a 100% improved condition but things have certainly improved with better airflow through the narrower pathway in the nose that was made larger.

Just recently, I visited the doctor earlier last month to check if conditions could be further improved as I wasn't sleeping as well as I wished to. After the doctor checked on my nose, he confirmed everything is good and there is no further surgery that is required after I inquired if there is any other form of surgery that could further improve on the condition. He only suggested to do a sleep study to see if I am suffering from sleep APNEA but I didn't go for it as the only solution is a CPAP machine which to me is not necessary. To me that machine will create more anxiety, more harm than good.

Long story short, I tried looking for suggestions on Youtube to improve breathing for better sleep at night after a post-surgery septoplasty. Interestingly, most of the videos are by ENT specialists themselves. It's more about massaging the face and doing throat exercises. I have tried some of the massages and throat exercises, and I find that massaging the face could somehow help a little in improving breathing. I'm still at the early phases but I think it's actually quite useful and could help alleviate the symptoms. Once you use the fingers to press against the cheeks near the nose where the deviated septum/narrower passageway which has been operated on lies, you will be able to feel that passageway clear a little.

The doctor suggested that weight is also a factor in contributing to the passageway of the nose and throat to collapse, thus leading to reduced airflow which disrupts or affects good sleep. If anyone is overweight, reducing the weight to ideal BMI levels would likely help as well. ALthough I'm not overweight, I"m striving to achieve my ideal BMI by cutting down about 6 or 7 kg.

Just recently went through FESS with Dr Raymond Tan Suan-Kuo at Beacon Hospital, for my frequently blocked left nostril. He mentioned using radiofrequency and performing septoplasty for my deviated septum. Total cost 21k +++ (Will include the final amount after I finish following up with him at ENT clinic)


Here are the chronological orders (and also writing a diary to keep track of my recovery process)

30/6/2026
Undergo surgery at 1 pm. Finish around 2 pm/3 pm. Wake up at around 5 pm after general anaesthesia. After surgery, he didn't give me a nasal pack to put inside both noses, and my nose is heavily bleeding. At this time, both noses are blocked

1/6/2026
Still heavy bleeding and many blood cloths inside both noses. Both noses are still blocked from breathing. Communicated this to Dr Raymond, and he said, "Don't worry, this is normal. It will get worse before it gets better" lol

2/6/2026
Still heavy bleeding. Visited Dr Raymond in the morning before discharge, as he wanted to pull the "Bei Chong" out from my left nostril. But not able to do it since there are still many bloods inside. He injected me with some antibiotics into my nose.
Discharged from the hospital at around 2 pm. Back home and use the nasal rinse repeatedly. The right nose can breathe again and feel better. But the left nostril is still blocked

Visited Dr Raymond again for a follow-up. Again, he wanted to pull the "Bei Chong" out from my left nostril. Same still failed as he said inside still many blood cloth, and couldn't find the "bei chong". And he doesn't dare to force pull or find it. Uses the nasal rinse repeatedly. At night, I can suddenly breathe through my left nostril. But my right nose is blocked now

Both noses are still bleeding, but not as heavily as before. The left nostril can breathe, but the right nostril is blocked now.

I don't know about FESS and only have experience with septoplasty surgery to address deviated septum as mentioned earlier in this thread. I will avoid being long-winded and get straight to the point.

Your experience will differ from mine as you have had both FESS and septoplasty while I only had the latter, and everyone's body and condition are different so experiences will surely differ. The doctor is right that after the condition after surgery is as what that you are currently experiencing, frequent blood running out from the nose and frequent rinsing and cleaning is required since the wounds need time to heal. Normally it will take days, in my case about 2 or 3 days before blood stops coming out from your nose every time you rinse it. However, breathing through both nostrils isn't blocked in my case and is like usual before surgery. Perhaps your condition is slightly different since you mentioned you had FESS.

Secondly, the path to complete discovery with septoplasty is a long one. Although blood has stopped coming out from your nose, the soft tissues or soft bones that are cut and removed inside the nose will need a long time to heal, and it will take months. I can't really recall but I think it took about 6 months or maybe more. Throughout this period of the healing process, there will be some nasty looking solid lumps that will be formed inside your nose, and it will be discharged out from your nostrils when you blow your nose. These nasty solid lumps when inside the nose smells very bad, and not only other people surrounding you will be able to smell it but you yourself will be able to detect that foul odour, and you may be wondering where does that smell come from. The foul smell is not from the nearby garbage bin but actually from inside your nose. When you blow it out, that thing looks gross and unsightly. This will periodically happen in say once in every week, or once in a fortnight until the tissues inside the nose are fully healed.

Usually after the surgery, the nurse will give you an ice pack to put on your cheeks near the nose to stop the bleeding. You need to do this right after surgery and at home after you have discharged until the blood stops coming out from your nose. The picture of the ice pack is as below. The cost of this item is included in the bill and is claimable by insurance. I am surprised that this was not provided to you after the surgery as I suppose it's a standard practice to have this included.

It's been day 6 (6th of July 2026) for me. Both nostrils are still bleeding lightly. And I notice there is some liquid keep dripping, but not as heavy as before
Sometimes the left side can breathe, sometimes the right side can. Before the surgery, the left side is frequently blocked. But now it's become the right side, which is frequently blocked lol. I've been using nasal rinse for more than 10 times a day

Yeah, I am surprised also that they did not give me the ice pack, not even a cotton to put into my nostril. When I asked for cotton, I was told it is charged, and I need to pay. Looking at my bills, even the waste bag (which is to throw the tissues that I used to wipe my blood) is also charged and included in the bills


Question, last time when you did septoplasty surgery with Dr Sushil, did you also do Turbinate Reduction? And when you resume your normal activities like going for exercise or eating with hot foods?

I looked through the bill and I presume turbinate reduction was included. I did not focus too much on the medical terms but recall the doctor explaining to me on the surgery procedures which are to correct or straighten the bone (septoplasty) and removal of soft bone or tissues, presumably turbinate reduction.

These are the main procedures described in the bill, and perhaps FESS was included I"m not sure. sweat.gif

- 100% anterior and posterior opening into FESS
- 70% anterior and posterior opening into FESS
- 65% excision of turbinate of nose (and bilateral) - presumably turbinate reduction
- 115% septoplasty of nose

Your condition of occasional or frequent blockage between the left and ride side of the nose is different from mine. Hopefully the condition will stabilise in few more days with both sides/nostrils of the nose having a cleared passageway. That is most important, to have minimal or zero blockage through all the passageway both left and right nostrils so that breathing is not affected at all times. In my case, my left side is always clear since the passageway is normal, and it's the right side that is mostly blocked (before surgery) as the passageway is extremely narrow. IF I use one finger to block the left nostril, I will choke to death as air is completely blocked on the right side of the nostril. If there is air going through the right nostril, it would be 10% to 20% only in contrast to 100% through the left side. I have actually lived with the condition for many years which amazed me, but decided to go for the surgery after I felt that the condition is affecting my every day life.

Glad that your situation has much improved and able to breathe normally now. For my case, before the surgery, I can only breathe in through my right nostrils since most of the time my left nostril is blocked. After I slept and woke up, I felt even more tired and dizzy. Now slowly getting better, and my left nostril can breathe now (But still in pain, and sometimes blocked due to the nasal mucosa still trying to heal itself, having blood streaks and thick scabs)

Glad that your situation has much improved and able to breathe normally now. For my case, before the surgery, I can only breathe in through my right nostrils since most of the time my left nostril is blocked. After I slept and woke up, I felt even more tired and dizzy. Now slowly getting better, and my left nostril can breathe now (But still in pain, and sometimes blocked due to the nasal mucosa still trying to heal itself, having blood streaks and thick scabs)

Yes, the procedures we have undergone are almost the same except that I see there is a procedure which I did not go through which is the Endoscopy of Sinus. That may explain on the a higher bill in your case, a grand total of above RM20k. The total cost of my operation is RM16k.

If you are entitled, your submitted claim will be eventually approved, it's just a matter of paying using your own money first which can be a hassle.

Also, our condition before operation is almost the same except that it's swapped between the left and right nostrils. biggrin.gif In your case your right nostril is fine and your left nostril is blocked. In my case the left nostril is fine while the right nostril is blocked.

A note is a septoplasty and/or turbinate excision is generally to make the passageway of one of the nostrils to be larger. However, there is a limit to how much the passageway can be made larger. In my case, based on the video and image scan of both my nostrils displayed on the screen in my ENT doctor's room, the passageway of the right nostril after operation is still smaller than the left nostril. Based on rough estimate, if the passageway of my left nostril is 100% in terms of area or air flow, the right nostril is perhaps at around 60% +/-. You can check on your condition of your nose with the subsequent follow-up visit with your Dr. Raymond after your nose has healed, and I presume the doctor will show you the images inside both of your nostrils after the operation.

Lastly, just a note which may or may not be useful to you or other people who have had gone through septoplasty. Just a sharing and this is based on my own personal experience. I think I've briefly described on this earlier but will repeat it anyway. It would be great the condition is greatly improved instantly after the operation, an immediate difference to the patient. However, I do not experience that to be the case. Even after 2+ years my condition did not really improve a lot. Although the passageway of my right nostril was already made larger, the breathing is still not as good as I had expected. I visited the doctor again 2.5 years after the operation which is earlier this year to confirm on my condition, and the doctor confirmed that condition is already good after he checked everything, my nose, throat and ears. All the passageways are fine, so I was perplexed why my breathing was still erratic which affected my sleep. The doctor only recommended a sleep study and suggested that I may be suffering from APNEA, and when I asked the doctor what could be done to rectify the condition if one is experiencing sleep APNEA, he mentioned the only method is to use a CPAP machine. After I investigated the thing, I said to myself no way I'm getting that to help me sleep better.

Long story short, after some investigation on Youtube, I found a remedy that actually improved my condition, and I can feel that my breathing through the right nostril has greatly improved now. It's massaging of the cheeks of my right side near the bridge of the nose where the narrow passageway has been operated on. I've been frequently doing it for about one to two months now and it's really effective. During the process one will be able to feel the breathing to be affected right there and then when the fingers pressed against the passageway. I am just surprised the doctor didn't recommend this and I have to go to Youtube to seek the solutions by myself.

Sorry this post got a bit long. For anyone who is still experiencing difficulties after an operation, do try to look up on the recommendations on Youtube to improve on your breathing. It doesn't matter if you have or haven't gone through a septoplasty or any nose operation. It can be a simple sinus issue or a blocked nose or whatever. Some of the recommended methods particularly the massaging of the face where the passageway of the breathing from the nose to the lungs lies, it is effective and can greatly help clear the passageway for a good improved breathing. In my case it had surely helped.
