If its defined as painkillers, then it shouldn't be an issue (Otherwise, Max Payne would've been banned as well)
Are you drawing a parallel based on what happened with Fallout 3, where they wanted to rename the drug names? (Which is quite silly to begin with because the first 2 Fallouts DIDN'T have real names either, so it was some stupid guy at Bethesda's fault for making that situation happen)
Most definitely its about violence.
Anyway, with Bioshock, you were injecting some "fantasy" substance into your body, not a real life drug.
