100k yen - 200k yen should be enough for a shopping spree and going around.
Average dinner per meal would be ranging from 100 yen to 1000 yen, depending on the places.
2 people would be 2000 yen+ maximum.

Which area in Central Tokyo are you planning to stay at?
There are lots of shopping areas in central, from Shibuya all the way up to Ginza.

In a way, why not the 2 night extra just stay at your gf place instead of a hotel?

As tanoshi and clemong had answered your question, I'll let in you 2 tips:
1. If its a JR pass that you want (as you didn't mention discount pass or anything similar), added to that you prefer to have 1 ticket than to buy again and again, buy the SUICA card at the JR ticket booth. Its like a reload card (not a discount card), which means for example if you buy a 2000 Yen SUICA Card, you can use it at any JR station with the same of payments for the travel, but only with 1 card (until the 2000 yen is finished, and you have to reload again at the ticket booth).

2. Press the English language button at any of the JR station ticket booth; almost 99% of them have it biggrin.gif

Several things to do while you are at Mt Fuji:
- Snap pictures and videos (bring extra battery and memory storage devices if you want).
- Take a rock sample from Mt Fuji high places. It was an active volcano a long time ago, so just take the rocks as a free souvenir.
- Write a postcard message and send it back to your house address. It's your official proof that you had been to Mt Fuji biggrin.gif (there's a post office there).

^ as you can see the route map of JR Yamanote Line, it goes to almost ALL the popular places in Tokyo!
Shibuya, Shinjuku, Tokyo, Akihabara, Shimbashi (go down here to take a train to cross over the sea to the Odaiba Island), Ikebukuro, Harajuku, etc!
And, its an unending circle route, which means that if you will not get lost anywhere since it will always be going around in the same circle! biggrin.gif
If you wanted to go to Akihabara but you overslept and thus overshot until the next station (lets say Kanda), don't worry!
Get down in Kanda, and take another Yamanote Line that goes the opposite way, back to Akihabara biggrin.gif

It depends from where to where you wanna go on Yamanote Line.
Cheapest (from one station to just the following next station) is just 130 Yen.
The most expensive (from one station to the totally opposite side of the line) would be around 250 Yen.

Not sure for an answer regarding specific details for accommodations.
Like I said before, for us who stayed in Japan, usually have our own personal houses/apartments.
Guess you have to wait for that answer to come from those who had been here on a visit biggrin.gif
