 Low heart rate and low HRV

Hi!

Just a quick question should i be worried.

I have a relatively low resting heart rate, which is around 41-45 bpm. But when I sleep it stools even lower in the 30s at certain points of my sleep.

I've also noticed my HRV also gets really low sometimes at 20ms-40ms during those periods.

Of course I've measured it using an Apple watch so it might not be as accurate (only managed to check the heart rate on a medical device which is spot on, once got held up in the clinic when I had a fever because my heart rate was too 'low)

A bit of background I'm active. I do exercise on a regular basis. 20s age group. I have no issues doing steady state cardio and intense workouts.

Just wanting other opinions before I go ahead and just meet a cardiologist. Had a medical check up done twice this year with blood work, everything came out fine.

I do have states of low energy and general sleepiness but that may be because of my sleep.

How active is active? Do you do a lot of intense cardio training?

If you do, then it's normal to have a lower RHR.

What med check did you do twice this year, were there any ECG test?

Apple watch is quite accurate, which model are you using?

How's your BP?

Blood test, lung xray, BP, Audiometric, bmi, heart rate, unfortunately no ECG.

SE model.

Last time I checked Iirc was around 120/70. Yesterday I hit a 158 bpm max on my workout.

Numbers seem fine right?

The highest I've ever gotten was 170 and that was 1-2 years ago. Used a bunch of other devices that monitor my heart rate when exercising, never went past 160

45-48 Iirc. Doc just asked whether I exercised regularly or not, in which I said yes.

Was prescribed antibiotic, unsure of what fever. But felt like I was dying lol. Recorded around 38. Suspected dengue but blood tests said otherwise. Had prolonged symptoms. My platelets and wbc were low so the combination of the tests and heart rate probably freaked the doctor out. Insisted I was fine.

Dad used to have an angioplasty and my grandfather had a heart attack so kind of sketched out by all of this. As for the AW. I turned on the irregular heart beat, no notifications at all. The only thing it kept saying was the low heart rate notification.

Based on family history, you have high risk

What were the medical tests metrics of your dad and granddad?

What was your activity when your HR was 170? What are you doing now that it's not above 160?

Can you try push your HR to 190 and notice how you feel?

When was your low blood count condition?

If you train cardio regularly (i train 10hours weekly and about 40 resting HR) its normal to have low resting heart rate among sport community. As our heart is a muscular organ, as you get fitter the amount of blood pumped per stroke volume is higher than an untrained individual hence fewer beats are needed to move blood in your body. Its a good indication actually that you have a strong heart.

Max heart rate is very individual actually no such thing "at this age you shud have X max heart rate and should push to around 190". That is terrible advise.

If anything you should do an annual check-up and not ask strangers for advise in a forum especially this is concerning your heart health with your family background.

I guess the 220-AGE formula is more suited for general public with 70+ bpm

But for those who has 40+bpm, I doubt the formula works since the one need to work up from 40+ to say 190 thats 150bpm jump compared to normal HR 70+, that only needs 120bpm jump.

Hence 160bpm sounds just nice as MaxHr...just my pov

i have the same HR as you

resting can be around 45-55 average and can reach 30s when sleeping for short period

done ECG in the hospital but they dont see any wrong

i'm not an athlete although i do exercise 2-3x per week

Low heart rate (Bradycardia) can be physiologically normal (i.e in Athletes), or it can also be due to various causes.. for eg. Hypothyroidism, certain heart diseases or even drug induced (beta blockers, etc..) Does the blood work includes thyroid level? Hypothyroid could cause all symptoms you mentioned (low heart rate, low energy), and its quite common. If not, u could have it tested first.

If u are still worried, feel free to schedule an appointment with a doctor for further check up.
