Found a possible solution, haven't got the time to test as I said few people call me nowadays. Pasting it here for people having same issues. Seems really a lot of hidden things for samsung.

I just switched from a 7 to an s21 Ultra. I had the same problem based on the set up it came with. Here's what worked for me.
Settings > Sounds and Vibration > Sound quality and effects (next to last option) > Adapt Sound (this is at the bottom of the page that has Dolby but is unrelated to whether Dolby is on or off) > choose either "adapt sound for media and calls" or "calls only" > Select the boost corresponding to age > also touch the little gear to the right of your age choice and chose my preferred ear for calls (left/right) and sound preference (clear).
There is also a personalized sound option based on a hearing test, but you need to wear headphones for the test.
