Err.. I didn't do anything special ya. Still normal application but my salary has increased quite significantly and hire purchase loan was already cleared off from CCRIS when I applied again in Oct.

Not sure if that helps.

No need. Online transactions are possible. Anything that you pay for a transaction is considered as a retail transaction.

Probably only restricted to those merchants while other cards follow MCC, so long the merchants are using that specific MCC then will be eligible.

That list is shared by fellow forummers, so if it's not there it could mean nobody has shared for that location. You can always try and share in the thread. For chain departmental stores like Parkson, they usually use the same MCC for all their outlets.

Usually the travel insurance comes with credit cards is not comprehensive. You are better off buying it on your own from Lazada for e.g.
