You have parental tax relief up to RM 1.5k per parent if you didn't claim parents medical care.

EA form is just one of the forms.
Unless you're not claiming back any relieves, EA form is enough.

Follow the annual statement provided by the insurance company.
Not 100% premium paid is claimable; some are meant for admin fee.

If I paid RM X for MEDICAL/LIFE (for GE IIP), I can only claim back 60% of RM X under medical only right?

The remaining 40% of RM X are either LIFE or MEDICAL right?

It is divided into 4 sections:

LIFE -> RM V
MEDICAL -> RM W
MEDICAL/LIFE -> RM X
OTHERS -> RM Y
TOTAL -> RM (V+W+X+Y)

Anybody filing for GE ILP?

Please help on how you guys file for MEDICAL/LIFE:

LIFE -> RM V
MEDICAL -> RM W
MEDICAL/LIFE -> RM X
OTHERS -> RM Y
TOTAL -> RM (V+W+X+Y)
