Accept credit card - Yes
Extra charges - No

Not sure about their dealers though.

Just to add, the One plan 128 (3GB) not entitled for the iPhone 6S Zerolution, so you're looking at 158 (5GB) minimum. That's RM100 (Phone Commitment) + RM20 (Zerolution Upgrade) + RM158 (Monthly Usage Commitment) = RM278.00 + 6% GST monthly.

Conclusion: Buy from Apple online.

I think all resellers and telcos sure void warranty for water damage. They will charge but I don't know how much - depends on your model and capacity.

Well, that's fine as long as the AASPs don't reject us when we go there - had a friend previously who bought his 5S from a telco and was told by the AASP to go back to the telco when the phone was faulty. But I think it's wise that telcos don't provide after sales support. It's not their core business and in most cases they don't have the capability anyway.

Usually if you are a postpaid customer and have good payment record, they will waive the upfront payment for a regular contract. However, if you are talking about the Maxis Zerolution Plan, then there is definitely no upfront payment - just a monthly commitment for the device + the plan of your choice. And I am pretty sure they can let you keep your prepaid number and switch it to postpaid.
