If you're from Elementalx kernel before, not necessarily to reflash rom but no harm in doing so. It just wipe your /system that's all.

It's cause by some app that didn't close properly that uses your camera. OS still thinks that that app is still using the camera, hence it pops this out when you open camera app.

Very very rare I encounter this problem. Like chances I get it is once awhile (maybe 1/50). Usually I just go to Settings > Mobile networks > Network operators and select my carrier again. Fix for me.

Are you behind any firewall? On your router or phone? Try going back to stock and see.

Since you're in US, I'd suggest you RMA it.

I also did some googl-ing around. Apparently some N4, also has this issue.

Yeah, sad case for you. Better rma while still can. Remember to lock back your bootloader.

Orange meaning that you got no connectivity with Google network. GCM are on ports 5228-5230. Make sure to allow these port from your router.

Have you tried with other routers? Eg: Starbucks, friend's house, etc.
