Basically everything in section B is taxable.

1) Combine them. Fill in the No Majikan with the highest contribution.

2) Gross and omit the cents.

Cannot. Because during audit, the LHDN officer will refer to your EA statement.

Your HR needs to amend your EA statement for you if those BIK is not taxable.

Some said that employer will forward/upload your EA form to LHDN office. I'm not sure if this is true.

You need to sort this out with your company HR/payroll.

You need to study your annual statement properly to determine how your premium is being allotted:

100% Life
100% Medical
60% Medical & 40% Life
100% Education

Add it up to be an annual sum between January 2014 to December 2014 I guess and apply a flat rate of 26%.

Sorry, I have no idea on this since I have no experience nor an accountant.

If you don't have No. Rujukan Cukai, you need to go to LHDN branch to register yourself since your company HR didn't help to create you.

Not subjected to income tax and filing are totally two separate matters.

EA form will guide you to fill in your e-filing.

I think short courses are not entitled for claim.

On top of that, your course needs to be approved by Ministry of Education and Ministry of Higher Education.

Only LIFE insurance policy and EPF contribution. This clause is very easily to be maxed since the max amount you can claim is RM 6k per assessment year.

If you have a investment linked life insurance policy, you need to refer to the annual statement to determine how much of the annual premium is being allotted for LIFE section.
