So if i buy NA or JP physical copy of FFXIV i need to register NA or JP SE acct in the same order. How do i pay for the subscription since malaysian CC is not accepted?

Yes if you buy an NA copy, register for a NA Account, vice versa. For CC well, there game time card and Crysta. Crysta is only supported in few country, one being United Stated & United Kingdom is the easiest to know. JP is also one of them to have Crysta Support.

Yes, please use same region PSN as the game you've bought, if EU, EU PSN, if NA, NA PSN.

Yell is short range, shout is entire area zone map. How short im not sure.

One time password is for those who bought the security token or download for free from Apple Store or Android store
