try DJFoo's method or use Camera ICS, download it from market. Basically it's a stock ICS camera app but you'll find a few more options if use this. one of the feature is mute camera shutter. Hope this will answer your question.

Bring it for replacement. I have not one Sleep Of Death till now. I've encountered it countless times on my old Desire Z so I know how it feels like. Seriously bring it back for replacement if possible. Confirm hardware problem. Mine's perfect.
