Shoooooooooooooot I haven't received my letterrrrr, anything important things need to take note of in the form?

Errrr if you guys didn't get any form, then how are you to know which camp are you sent to, and which venue to gather to get on your bus?

I haven't receive my letter,can I just download the medical check up form at http://www.khidmatnegara.gov.my/?page_id=52 ? and go to nearest hospital to perform the check up?
