One pair from Port 1 (or 2 or 3) from your DIR-615 router to your PC for internet, and one pair from Port 4 (the red colored port at DIR-615) to your STB. But you have to pair the homeplug separately, i.e. they have unique SSID for each pair (say first pair with SSID of Internet and 2nd pair called IPTV).
