I own 2 atm cards now and I don't need to pay any annual fees. So, I feel like paying fees is like milking us.
Most ATM cardholders in Malaysia do pay annual fees for their ATM cards.

Maybank and Citibank are the only two banks that offer free VISA debit card
(and also debit Mastercard for Maybank). Citibank Citibanking customers however, will have to pay one time issuance fee of RM8.

That's because he/she is using Basic Savings Account. As far as i am concerned, EON Bank is the only bank that charge ATM card annual fee to their Basic Savings Account holder.

1 is Maybank, 1 is from HSBC.
I believe the HSBC account is a Basic Savings Account but please bear in mind you are only allowed to make 8 free withdrawals in a month and subsequent withdrawals will attract a penalty of RM1.
As for Maybank, you are only allowed 4 free withdrawals per month and subsequent withdrawals you will be charged RM0.50.
