No problem in using a malaysian iTunes account. The only drawback to this is you can only access local content i.e only apps from the appstore. Even then, you won't find all apps are available in our local appstore. Some apps are country specific. So if your main use of iTunes is getting apps thats available locally then by all means register the malaysian account.or should i just follow the rule and create Malaysia account and put my credit card number?

But if you want to access all content i.e all apps, songs, movies, tv series etc, then you need US iTunes account. The only drawback is that you need to top it up with iTunes Credit. You can't link the account with our local CC (but if i'm not mistaken, they also take paypal).
About the debit card, I don't think iTunes accept debit card.
