i had run my own bizness for the past 9 month.. and i got myself MAjikan EPF number and also register as a enterprise at suruhanjaya..
but the question i wan ask is " how i get NO.Majikan" and do i had to print out EA form to my staff?
but i got no No.Majikan.. u know where to register it ? and EA form format where can u get it ? appreciate someone tat could help.. thx..

No Majikan should be your company registration number. If sole proprietary should be your license business number in your Form D from SSM

EA form format isn't fixed. My ex company does their own format. Besides, LHDN doesn't even want to see your EA form, unless they audit you and others individually...but usually with ample time for you to prepare your docs for audit.

EA form is for yr workers only. no need submit to IRB one. but EA form got new format oredi. can download it from www.hasil.org.my

so are u sure is my company register number? but i had ask some of my frnz.. they said not the Form D register number ohh... or do i need get tat special number frm LHDN?

Colin, Nombor pendaftaran majikan , you will need to apply from LHDN.
There will be a form to fill in and documents to be submitted to LHDN.
The number will start with E xxxxxxx-xx
You need this number to pay PCB
You need this number when you file your Form E
Form E used to be summary of EA Forms

Yes is true. But you need to prepare the EA Form for your employees for them to submit their tax return.
By the way, if you don't do the EA form, how are you going to fill in the Form E.?

Did u guy notice that Form E for 2008 is very easy to fill up. Compare to year 2007 which you have to state every month total gross salary.

I understand that this is an old post but any future forumers can learn from the tips below.

Most tax payers are not sure of what numbers to fill in the sections as below:

Insurance nyawa dan KWSP........................................................Terhad 6,000
Skim Persaraan Swasta dan Anuiti tertunda (‘Deferred annuity’)......Terhad 3,000
Insurance pendidikan dan Perubatan............................................Terhad 3,000

All you have to do is just Refer to your Life Assurance Premium Certificate(LAPC) to fill in the numbers.
