how to get No.MAJIKAN tat stat on EA form?, EA FORM issue

i had run my own bizness for the past 9 month.. and i got myself MAjikan EPF number and also register as a enterprise at suruhanjaya..
but the question i wan ask is " how i get NO.Majikan" and do i had to print out EA form to my staff? but i got no No.Majikan.. u know where to register it ? and EA form format where can u get it ? appreciate someone tat could help.. thx..  |
