I understand that this is an old post but any future forumers can learn from the tips below.

Most tax payers are not sure of what numbers to fill in the sections as below:

Insurance nyawa dan KWSP........................................................Terhad 6,000
Skim Persaraan Swasta dan Anuiti tertunda (‘Deferred annuity’)......Terhad 3,000
Insurance pendidikan dan Perubatan............................................Terhad 3,000

All you have to do is just Refer to your Life Assurance Premium Certificate(LAPC) to fill in the numbers.
