and if u using us acc, then you need us cc or us paypal to purchase the paid app

whereas MY acc, u can just use your MY cc/paypal. Besides that, MY itunes acc currently only touch app, there's no music/video and etc..

Got difference 1... US iTunes supposedly has a FAR wider choice of apps and games compared with the Malaysia one... And u are required to have a US credit card to open an account with US iTunes...
