There is another posibility is due to your RAM timing. Did you overclock your system?
I didn't overclock my RAM, but I run it at my OCZ's default timing which is 4-4-4-15 @2.1V. However, now I already lowered them to @1.8V with loosen timing 5-5-5-18

Incompatible, conflicting or missing device drivers perhaps. Very often, I could simply determine if this is the failure of hard disk and if so, if it is a logical or physical failure.
Hopefully it could be only one culprit and not more than one

. Perhaps I should get the MemTest86+ to test my RAM
