ISP handle the connection and bandwidth routing
VPS provider/your own server handles the firewall and traffic filtering
and you can put service like cloudflare in front to prevent the traffic from even reaching
and yes, when you put your server to public, you should be handling the traffic via firewall or front it with some service like cloudflare to prevent DDOS
