Brown spot after painting, Paint

For context, its a 30year ish house
Just did a paint job and brown spot appear after the paint dried I googled, it says water stain/mold/mildew issue, but not much solution available. How do i get rid of it? Repainting it with new layers doesnt help much Whats the recommended prep work to do in the future for this kind of issues? Check the moisture content of the slab first. If over 20%, it is wet internally.
