Dear All experienced sifus and helpers

I am disabled with my left eye rendered blind due to a Hit-and-Run accident many years ago bud ever since the left eye is not functioning. I wish to apply for OKU status. Can someone, or volunatary organisations, please guide / advise me on how to go about and the time frame of getting the OKU status. I'm staying in Subang Jaya. Thanks, God bless and take care.

You can check YouTube OKU card apply

get/print OKU form.
make appointment with doctor mata kat gomen hospital, the specialist will sign the form to clarify/confirm ur disability...

p/s... bila form dah sign and chop by specialist, baru pi submit kat pejabat OKU. kalau kat KL, opposite Hospital Besar KL ada pejabat OKU.

Can also go private specialist clinic.. will also be accepted by JKM.. application form need to be submited by hand.. then they will inform if approve and ready for collection

Go ss15 got pejabat kebajikan there can apply Oku card. Go gov hospital first got a form they will help u fill one

if not mistaken now u can apply online via JKM website

I visited a doctor to get a medical certificate confirming my disability. Then brought everything to the JKM in SS15. Was told it would take 4 weeks but took 8 weeks during the pandemic.

Had to get a replacement card recently and it took less than 2 weeks.

Those in PJ can go Subang SS15 pejabat kebajikan masyarakat to submit the OKU application. Bring 1 photo and 1 IC photostat. Form can be printed online in JKM website. Form can be certified in hospital. In my case I got it certified in UMMC jabatan maklumat. It's a hassle to certified in UMMC since you can certified outside according to this thread you can do it outside.

I'm an OKU and registered in person in SS15.
It took about 8 weeks during MCO but should be much faster now.
