Those in PJ can go Subang SS15 pejabat kebajikan masyarakat to submit the OKU application. Bring 1 photo and 1 IC photostat. Form can be printed online in JKM website. Form can be certified in hospital. In my case I got it certified in UMMC jabatan maklumat. It's a hassle to certified in UMMC since you can certified outside according to this thread you can do it outside.
