Regarding that 3 yrs start to claim sure can get approved....
I think, they may approved that admission and gives out GL and also discharge bill.....
After that. I think, the insurance company will study the claims and hospital reports and if they suspects or think that it warrants more indepth look, I believes they will starts their own investigation....perhaps visits clinics/hospital near the place of residence and workplaces for any medical records of said person.
Or even the death record or medical history of immediate family members to see if they are any similar illness, if there are and if it was not declared when ask, then could be a non disclosure of material facts.
(I remembered when I bought last time, I was asked when my parents die and what were the reason.)
If no issue than OK...
If found to hv non disclosures, like example, you stated your family members has no medical issues but later if the insurance company found out that some of them has cancer and you are the one paying for their treatment or doing the admission/Discharge paperwork process or signing documentation for that hospitalisation, then..maybe they will take action., perhaps cancel the policy and returns some of the premium paid.

So even if one start to claim after 3 yrs, ...GL cashless admission approved, but the company i think can still investigate if the wanted too.
Perhaps for certain illness,...i think they will not but if for some Chronic disease issues that usually manifest slowly or has early detectable systoms or illnesses that needed long term treatments and will cost alot each time, they may..
Better to spend few k to do the investigation than do nothing and see frequent "potentially suspicious" claims payout. Confirm it than it will not be suspicious. Ha ha
