At age of 30 around 8.5-9k only. Only when they jump to private there is a huge huge jump

The options for graduates from local private and some public universities are limited - Singapore recognises only UKM and UM, and only since 2007. Twinning degrees eg as offered by IMU are not recognised. SOURCE

Actually anecdotally I have lets see 3 schoolmates who studied medicine there and serving there. I believe they took the Asean scholarship route and on scholarship for A levels.

But, the vast majority of applicants for medicine are locals. There's less than ½ a dozen places allotted for Malaysians wishing to pursue medicine at NUS per cohort of 300. That's how competitive it is.

Yeah but transition not as easy as you think unless it's related like electronics to software. Also demand in engineering field change roughly every 5 to 10 years. Civil -> Electronics -> Chemical -> Software(?) -> ? Lack of doctor job mostly due to abundance of medical degree but not to the extend of engineering degree.
