You should receive an IPA after you sign the offer letter and the company submits the application. The IPA will indicate what kind of medical test do you have to undergo.
What I did was, sign the offer letter, informed the new company that I will only tender my resignation upon granted the IPA. Upon receiving the IPA, i tender my resignation.

If they do background check and found something not right, you may not be granted approval. But IPA means, it appears ok, so say u fail your medical check-up...the application will still be rejected. But i'm not the expert in this matter, just sharing my own experience.
