Yes, it applies to all banks as well. One card = one person.

Yes. Some banks extend their PPL access to their supplementary card holders too. I know AmBank is one of them and with separate quota.

PBB and Citibank Premiermiles also extend to supp holders, but PBB has shared quota of only 3 or 5 times depending which card you hold.

It is a combined quota with the other VI, if I'm not mistaken. Others can correct me.
