For deposit, yes forms for multiple topup but banks can catch the units first then print the forms continuous.. For withdrawal one form enough (I withdraw from ASG-kesihatan, persaraan)

Never encounter such things. Even did topup at post office in multiples of RM250, no such forms.

Scenario
Staff don't allow you to leave the book at the counter. Need to queue/camp at bank/visit the bank regularly to get.
So tell the staff I help you, you help me. Leave the book at the counter for staff to try and staff dapat money. Both side happy.

Most likely bank staff. Is actually illegal but hey by doing this, he/she confirm get units. So no need to worry cannot get units/fight with people for units.
But then again, some of the people here do the same but without paying anything so this is not illegal.
