I believe the payment gateway is determined by the location of the merchant/store. I spent on oversea store, all not MY.
Lazada MY used to be Paypal SG hence 5x, until recently it become Paypal MY then 2x.
If you send money abroad which ur Paypal MY account, regardless currency it is still 2x for my case.

Regarding Amex, it is not allowed if you are spending MYR, if you are spending foreign currency, then Amex is guarantee for 5x.

M2Premier's Amex doesnt has free access, only the VI entitle for 5x.
