I declared 2012 tax.

But for 2013, i worked from 24 Dec'12 to mid of Sep'13.

Then I started to work in SG Nov'13 till now.

Did I need to declare? I dont think i had EA form.

Yes, off course you need to declare your income for 2013 albeit till september.
if you don't have your EA, you need to add up your income via your pay slip

I dont have EA form, I add up my income b4 deduction of EPF or after?

Aggregate income is your gross income inclusive of all fixed allowances and OTs.

EPF deduction will come later under relieves with life insurance up to a max of RM 6k
