Check on your silver plug. The plugs are silver plated and not a solid piece of silver so once the plating wears, the plug is useless.

Depends how large your loop is actually. But people usually just put one.

Yes, flush. Some manufacturers don't have clean processes and leave flux or crap behind after the piece is finished.
