This bank is known of doing this. My friend apply home loan also sneakily give him a credit card.

normally, there are few pages that NEED your signature/consent before bank take ACTION (to subscribe/to charge)

it is your own responsibility to READ all the pages, understand it carefully, asks questions, etc before signing it.
feel free to argue with the bank.

if filling forms, normally (if possible), i would just cross-out the whole section too. this is to prevent staff from fake signing that section if you leave the section unmarked/uncrossed. it's a b&w proof that we did not agree and consented to it.

If you filled the form digitally and sent to them, you have your copy to prove if the staff added anything. Just inform the bank on that fact and the problem will magically disappear.

Have not applied supplementary from RHB ever, but I'd be surprised if the default form has 'built-in' that you need to opt out. Definitely manipulative and will trigger complaints to Bank Negara. Another possibility is that the bank officer 'help' you to check an option.
Definitely everything must read before signing. I suggest you bang table at the bank and ask why there's an option that you didn't select yourself. Ask them to show you the form and then see if its actually 'default' or marked. If marked, then some bank officer tricked you, ask for action to be taken.
