Matr1x kernel has -100mv undervolt by default, its likely your unit couldn't take the undervolt and reboots as a result.

This is the standard gapps package that one flashes for most ROMs, the only exception is for ParanoidAndroid ROMs you'll need to flash that particular ROM's gapps package instead.

Btw, speaking of H+. I'm using DiGi and I've never seen the H+ sign no matter where I go, and I don't think DiGi even had HSPA+ in the first place.

Good ROM hmm.gif I'd still suggest trying out some till you find one that you prefer. I'm using PA for example.

That is the camera's autofocus mechanism. It's not a problem as all N4s as far as I know have this, its just part of the phone I presume.

I'm not too sure about the camera, cause right now my own N4 does not have a rattling sound when shaken, but it does if i hit it against my hand.

Try changing kernels to see if that solves your problem, sounds like said kernel / kernel settings you did could not step up in time.
